Taylor Made Academy Day Camp Information


Co-Sponsored by FC West & Banks Youth Soccer Association

Forest Grove, Oregon @ Lincoln Park Athletic Complex on Pacific University

July 28 - August 1, 2008  9:00am – 2:00pm

$140 Ages 6-14

Academy:
(Ages 6 -14) This camp features five training sessions in an environment that is a combination of high level learning with fun activities and competitions. Players will be challenged to improve the skills needed to compete at the higher levels of youth soccer. TaylorMadeSoccer Camps will be utilizing current Division I players from various collegiate programs from around the country to provide demonstrations and instructions. Each camp will be overseen by one of the current Executive Directors (Dana Taylor, Paul Meehan, Jim Cofer, Gary Hamel) bio"s are located on the staff page.

Camp Information

Medical Consent & Release of Liability:
Each camper MUST fill out a Medical Consent & Release of Liability Form
Campers will not be permitted to participate if they do not have a signed form
A form can be downloaded and mailed to:

Registration/Confirmation:
You will receive a confirmation letter via email  once you have registered on line.  No later than two weeks prior to camp, camp directions, times, and a "what to bring - camper checklist" will be posted on the camp web site.

Gifts:
Community campers receive camp tee shirt and ball.

Tuition/Payment:
Visa, MasterCard accepted through online registration ONLY.

Cancellation Policy:
If canceling within 14 days prior to the start of camp the entire deposit will be forfeited to cover costs. A $50.00 fee will be assessed for cancellation of any kind. Refunds take 4-6 weeks to process.
